Payment is due when services are rendered.  We accept a variety of insurance plans.  As a courtesy, we will submit your insurance claim for you in our office.  You will be responsible for paying the copay (if any). 

For your convenience, we accept a variety of payment options, including Cash, Visa, Mastercard, Discover or American Express. 

Financing options are available through CareCredit^® and Chase Health Advance^sm.  You may apply for financing in our office or online. 

If you pay the full balance for your treatment in advance utilizing cash or a major credit card (not including CareCredit or Chase Health Advance) we may provide a 5% discount (for treatment above $500.00).
